Forms  forms

Upon entering our office, our receptionist will welcome you and get you started with our new patient paperwork. We will request that you complete our new patient forms before we get started.  This paperwork provides us with your health history and information on your condition.  If you would like to save time you can print off some of our paperwork here and complete it before your initial visit.  We verify any insurance you would like to use at the first visit as a courtisy to our patients but because of difficulties reaching insurance companies that can take up to 24 hours.  If you would like to have your benifits verified before your visit you can fax a copy of your insurance card and drivers liscense to 678.494.9771.   Insurance is not required to see us at our office in fact many patients choose not to use their insurance and pay cash.

Consultation  consultation

Next, you will have a consultation with doctor to discuss your health-related problems, concerns, and potential treatment options.  Our goal of the first visit is to find the root cause of you ailment and decide if we can help you or if a referral to another specialist is warranted.

Examination                                      Chiropractor examines neck

After your consultation, the doctor will perform a complete chiropractic examination testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests will be performed as well. However, nothing will be done in our office without your consent.

X-Ray x-ray

Depending on your specific condition, we may take X-rays as well.  Our office is pleased to have X-ray capability right on site and we will be able to take X-ray films right in the office if deemed necessary. X-rays help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. Most people are amazed once they see their X-rays up close. In some cases, they can even identify their misalignments and degeneration themselves.

Treatment  Woodstock Chiropractor Servicepic

As stated earlier the primary goal of the first visit to find the root cause of the problem.  In many cases we will begin therapy or treatment in the office to achieve some relief before the end of the visit.   A full treatment will generally be researved for the second visit once all the information including X-ray’s and exam has been fully evaluated.   At the end of the visit the Dr. will generally give some at home instructions to fallow until the next visit such as using heat, ice, exercises to begin or activities to avoid.

What Next?

At the second visit we will perform a report of findings sitting you down and discussing our findings and how it is affecting your health including prognosis and expectations.   Many people choose to bring a family member or spouse to help obsorb all the information we encourage this but it is certainly not a requirement.
